This link will take you to a questionnaire that will request your list of prescription drugs,  the pharmacy you prefer to use, the  doctors you see and a Scope of Appointment .  This is the starting point to our plan review. This will also allow us to narrow down your options that are specific to you, so your time is not wasted.

By providing your current list of prescriptions, this will allow us to find the drug plan that will be most effective plan for you. It will also allow us to find the plan that will work with your pharmacy. 

By providing your Primary Care Doctor and Specialist you see, this will allow  us find the plan that works with these providers.  This will also allow you to make an informed decision on the plan you pick based on the networks your doctors participate in. 

The Scope of Appointment is a Medicare requirement. It doesn’t change anything or commit you to anything, it just says you are allowing me to discuss medicare with you. This form will document what plans you would like to discuss. It must be submitted 48 hours PRIOR to our meeting.
